Clinical Pearls
- Using a distal transparent cap on a standard gastroscope is an excellent technique for flattening duodenal folds, dramatically improving the visualization of an elusive or hidden major papilla.
- The cap allows the endoscopist to gently manipulate the periampullary tissue, providing a stable, head-on view of the papilla that is often critical for difficult cannulations or identifying subtle ampullary adenomas.
- When standard forward-viewing or side-viewing scopes fail to provide adequate exposure of the papilla due to severe angulation or redundant folds, deploying a transparent cap is a simple, cost-effective rescue strategy.
